What makes up a taxi fare?
Base fare, then distance and time rates, then extras — tolls, airport fees, booking fee — and finally tip. Metered cabs print the rate alongside each part.
Should I include pickup and drop-off?
Most expense policies want them. Both fit in the two-column details block, above the fare lines.
Is it free to use this template?

Which formats can I download?
